The First day of School

■ 2 playground areas for Recess (K’s have their own!)

■ An Early Literacy Intervention support model

■ Rich cultural arts programs and community partnerships

■ Lots of opportunities for parents to become involved & a generous PTA!

Nurse: Jan Rahn# 1 - IMMUNIZATIONS (DTAP, Polio, MMR, Varicella/Chicken Pox )- Must be received by AUG. 31 If not, your child will be excluded from school until received. This is a state mandated policy from the MDPH.

# 2 -ABSENCES/TARDIES & VACATIONS- Please call the absent/nurse line @ 508-647-6571 or call the office 508-647-6570 or email jarahn@natickps.org

# 3- MEDICATION POLICY: An Authorization Medication form MUST be filled out by parent and doctor for any OTC medications and prescriptions.

If a prescription drug is to be given; it MUST have the pharmacy label with child's name on it, the name of medication and dosage. Have the pharmacist put it in 2 bottles (for home and school) ALL MEDICATION MUST BE BROUGHT IN BY PARENT.

If your child is going to be on medication next year-please request the Medication Form from the nurse before the start of school.

# 4- Pupil Emergency Form--Sent home at the beginning of the year. Please fill out BOTH sides ASAP and return it to school. We need your contact information and your emergency contacts.
