• Please fill out the IN-113 form if you received any of the following from a different state (other than VT):
- 1090
- 1042S
- W2
Note: If applicable, Form IN-113 has to be filled out before filling out Form IN-111 (see instructions in subsequent pages).If you earned income in more than one state, you need tofile a separate tax return for every state.
• Otherwise, please fill out the IN-111 for all Vermont State Tax
return. You will find the tax return forms here:

Things needed for State Tax Filing:
Black or Blue pen (Write in capital letters on all forms)
Social Security or Individual Taxpayer Identification Number if
you have either
Federal Income Tax Return forms (1040NR-EZ or 1040 NR)
W2, 1042-S, 1099, etc.
You may also be asked to provide identification at some time.
Notes: If you earned more than $100 for year 2016 as a state resident, you are required to
file a Vermont Tax Return. If you earned $1,000 or more as a nonresident you must file.
Please carefully read the instructions on the form and on this PowerPoint.
The instructions on the form (subtract, add, etc) will help you to fill it out.
Assume all form examples s are for the new year – 2016.
