Malignant melanoma in an elderly patient: casereport

A 68-year-old man developed a malignant melanoma at thesite of triamcinolone injection for keloid scarring.

The man had a history of a malignant melanoma on his rightupper back treated with wide local excision. Five years later, adermatologist observed a lump at the site of the melanomaremoval, consistent with keloid. A one-time dose of 0.5–1.0ccof triamcinolone 6 mg/cc was injected subcutaneously at thesite. Over the next 2 months, the region steadily grew, and asubsequent biopsy revealed a 3.5mm residual malignantmelanoma with angiolymphatic invasion. There was noevidence of metastatic disease found on CT scan. The man wastreated with wide local excision and followed closely withfrequent visits. He developed metastatic disease to the bones,lungs and musculature over the next 4 years, which respondedwell to radiation, chemotherapy and immunotherapy.
