• CHECK-OUT FORM NEEDS TO BE FILLED OUT AND SIGNED BY EITHER THE ADVISOR/DIRECTOR .

• A DELEGATED USER FORM NEEDS TO BE ON FILE WITH THE BUSINESS SERVICES OFFICE.

• MUST RETURN THE CARD BY 4:30 P.M.!! !!

• FAILURE TO RETURN CARD, WILL RESULT IN THE USE OF THE CARD BY YOUR ENTIRE DEPARTMENT DENIED.

AT THE SAM’S CLUB SELF

CHECK-OUT.

If over 11 items must use regular check-out. Charge it out as tax exempt, when they ask for a

PO number give them your index number without the A.

You will be prompted to pick Taxed or Tax Exempt.

It will then show on the top of the screen in red, that you will not be charged tax.

Scan your items…

When you click the pay option the screen on the right will appear. This is where you will enter your index number. DO NOT INCLUDE THE A! (Ex. A12345 you will just type 12345)

Sign your name and grab your receipt.

RETURNING THE CARD TO BUSINESS SERVICES

WE WILL NEED THE FOLLOWING:

• RECEIPT WITH SIGNATURE AND INDEX NUMBER WRITTEN ON TOP.

• MEALS & ENTERTAINMENT FORM (IF NEEDED).

• THE SAM’S CLUB CARD!
