The nursewill triage after-hours calls from patients, make home visits toassess and treat patients, schedule patients for clinic visits andwork in the medical clinic. The nurse will be on call an averageof 7 days per month, sharing call with three other nurses.

The nurse will join a clinical team that includes two RNs, threeLPNs, one NP and one MD. The medical team is part of a largerinterdisciplinary group that works together to care for olderadults with chronic illnesses who live at home and need helpwith activities of daily living.

Along with the physician, nurse practitioner and registerednurse, the interdisciplinary team includes a dietician, pharmacist,social worker, physical therapist, occupational therapist, homecare coordinator, transportation specialist, recreation directorand PACE center manager.
