The Optum Alaska Case Management team includes Care Navigators, a Senior Wellness Coordinator, and a Peer Support Specialist.

Care Coordination and Advocacy is the primary function of the Case Management Team.

The Case Management team works closely with participants to understand their physical, behavioral and social/environmental needs.
