• SAVED clinical data is NOT COUNTED as SUBMITTED.
• Facilities can search for patients with “Missing Clinical Values” and “Patients with Saved Clinical Values.”
• Double-check data entries – You cannot change data at the facility-level after a reporting month is “Closed.”

• The “Post-Dialysis Target Weight for Session” field is not asking if the target weight was met.
• The “Assessment Date” field should contain the date of a patient's last formal post-dialysis weight assessment (if prescribed).

Hepatitis B surface antibody (anti-HBs)
• Indicates the patient's hepatitis B surface antibody lab level.
• DEFINITION: “An antibody to the surface antigen of the hepatitis B virus; indicative of either active immunity to the hepatitis B virus as a result of prior infection or passive immunity from the presence of hepatitis B immunoglobulin in the blood.”
• Previous label: Anti-HBs (Antigen to hepatitis B surface antigen).

No Clinical Data Available For All Collection Types
• Allow the user to indicate when no clinical data (HD, PD, or VA) is available for a patient.
• To be used by facilities if a patient appears on the Clinical screens in CROWNWeb for a month in which the patient was not treated.
• EXAMPLE: A patient passed away on June 2, 2014. His name will appear on the June reporting month, but no treatments were rendered in June.

UKM or Daugirdas II
• Indicates the method used to calculate Single Pool Kt/V for Kt/V Hemodialysis.
• UKM (Urea Kinetic Modeling) and Daugirdas II are the only acceptable methods of calculating Single Pool Kt/V under the ESRD Quality Incentive Program (QIP).

Access Type Definition
AV Fistula Only (with 2 Needles)
TWO needles placed in the AV Fistula during the dialysis session. Fistula refers to a surgical connection between an artery and a vein just beneath the skin of an arm or a leg.
AV Fistula Combined with an AV Graft
ONE needle in an AV Fistula and ONE needle in an AV Graft during the dialysis session. Fistula refers to a surgical connection between an artery and a vein just beneath the skin of an arm or a leg.
AV Fistula Combined with a Catheter
ONE catheter lumen and ONE needle in the AV Fistula is used during the dialysis session. Fistula refers to a surgical connection between an artery and a vein just beneath the skin of an arm or a leg.
AV Fistula Single Needle Device
ONE needle in an AV Fistula during the dialysis session. A single needle with the fistula access may include a single needle for single access or a single needle for double access.
AV Graft Only (with 2 Needles)
TWO needles were placed in an AV Graft during the dialysis session. Graft refers to a piece of artificial vessel that can be used to create an access; one end of the graft is connected to an artery, the other to a vein.

AV Graft Combine with a Catheter
ONE catheter lumen and ONE needle in an AV Graft is used during the dialysis session. Graft refers to a piece of artificial vessel that can be used to create an access; one end of the graft is connected to an artery, the other to a vein.
AV Graft Single Needle Device
ONE needle in an AV Graft during the dialysis session.
Catheter Only
Dialysis treatment provided using only a Catheter. Catheter refers to a tube that is inserted into a body cavity, duct or vessel; allows drainage or injection of fluids or access by surgical instruments.
Port Access Only
A valve and cannula that subcutaneously implanted and is accessed by dialysis needles.

• Click the Clinical tab from the Homepage.
• Select the Facility DBA, Collection Type, and Clinical Month.
• Select “With Saved Clinical Values” or “Without Clinical Values” in the Display Patients list.

Can you demonstrate when to use the Infection N/A checkboxes?
• CROWNWeb presents two means of allowing users to check “N/A” in the Infection section:
1. If the patient does not have an infection.
2. If the patient has an infection, but information regarding if it required hospitalization is not available.

Should the Pre-Dialysis Weight and Post-Dialysis Weight entries be from the date that labs were drawn?
• Pre-Dialysis Weight: Select the patient's pre-dialysis weight when pre-dialysis BUN was drawn, in either pounds (lbs) or kilograms (kgs) in NNN.N format.
• Post-Dialysis Weight: Select the patient's post-dialysis weight if post-dialysis BUN was drawn, in either pounds (lbs) or kilograms (kgs) in NNN.N format.
