Bring with child:

Child’s immunizations

Child’s birth certificate

Parent/Guardian’s drivers

license

Proof of residency

Monroe County Health will be on site to administer hearing/vision screening.

The Kindergarten staff will be here to administer the Kindergarten assessment to the students.

Must be 5 years of age prior to 09/01/20

Waivers available for children turning 5 between 09/02/20 and 12/01/20.

Custody paperwork (if applicable)

Proof of income (2019 W-2, pay stub, bank statement, etc.)

Must be 4 years of age prior to 09/01/20

Waivers available for children turning 4 between

09/02/20 and 12/01/20.
