Step 2: Click the COVID-19 Updates Button (Red)

Step 4: Once selected you will land on the Employee Exposure Hotline homepage. If you’re having trouble arriving to this page, contact the LCMC Health IT Help Desk at 504.702.HELP.

Step 5: Select Take me to the vaccination page

Step 6a or 6b: a. To update your vaccination status. Select Make new entry, attach your vaccination card, and input your vaccine information.

b. To file for a vaccination exemption. Select Click here for exemptions.

Step 7: Once you select Click here for exemptions, download and complete the appropriate medical or religious exemption. Follow steps 1-4 to upload your exemption.
