*Ifyouforgotyourpassword,youwillneedtoresetit.Ifyoudonotknowyourusernamethenchecktheboxthatsays,“IdonotremembermyUsername”.The“ResetPin”isgoingtobethelastfourofyourpatientIDnumber.Youwillthenbeprompttoeitherresetyourpasswordand/oranemailhasbeensenttoyouwithyourusername.*

StepSix:Youwillcometoascreenthatlookslikethebelowpicture.ProceedtoeachpromptuntilyouhavepurchasedyourIDcard.
