How to Complete the Personal Health Assessment

(PHA)

1) Go to www.lmh.org and click on the LMH WellCare link on the top RIGHT hand side of the screen.

2) Review the information under the Welcome City of Lawrence Associates and then Click on the Sign In

with Cerner Health icon on the BOTTOM of the screen.

3) If you already have an account login with your username and password.

If you have never signed up for an account enter your email address on the RIGHT hand side of the

screen under I Need an Account, and then click on the Green Sign Up button.

4) Complete the Create Your Cerner Health Account information.

5) Click the Approve button on the following screen.

6) Review the Program Agreement page and click on the I agree box. Then enter your CIGNA ID

number from your CIGNA card and click the Enroll in Program button at the BOTTOM of the screen.

7) Click on the View Your Dashboard button

8) Click on the Start Button under the Actions.

9) Begin taking the Personal Health Assessment!
