Wellness and Nutrition Services This survey form will collect information about your school’s health programs, nutrition programs, physical and health education, and wellness policy.

Healthy School Act 2010 (Online Survey Form)

Log In DIRECTIONS:

• Visit https://octo.quickbase.com. • Select the “OSSE School Health” database. • If you have a “dc.gov” email, sign in with your network email address and password.

Main Menu: To access the Healthy Acts School Survey Form

• Click link “Healthy Schools Survey”

• To get started click “Survey”:

Open a page in form view or edit. • This survey has 3 pages. • Editing allows you to enter your responses. • Viewing allows you to see the form as it will be submitted

• Respond to all questions:

(use the scroll bar to navigate up/down the form.)

“Pages Completed” field indicates page status. 3 stars denotes the form is ready for submission.

• When you complete a page, check the completed box and save the form.

• The form will change to “View” format. • Click on the link to proceed to the next page (Go to Page 2

).

• Click “Edit”

• Continue entering responses on Page 2 • When you complete a page, check the completed box and save the form.

• The form will change to “View” format. • Click on the link to proceed to the next page (Go to Page 3

).

• Click “Edit”

• Continue entering responses on Page 3 • When you complete a page, check the completed box and save the form. • Follow instructions for Page 1

• Once you have checked all pages as “Completed” your survey is complete! • You may save this form at anytime and return to it. • You have print and email options. Click and follow prompts.
