Harvest Super User Tasks Nicole Carter & Clair Smith

Orchard Software Corporation June 21, 2018

1. Identify and describe password reset and general emergency security settings.

2. Configure database tables: Quick Comments, Text Shortcuts, Lab Tests, Order Choices.

3. Discuss enhanced QC file enhancements: Rules and setting up a new lot.

4. Recognize how rules help manage laboratory processes.

Learning Objectives

Emergency sign in allows specified users to temporarily be granted elevated security access. It allows an administrator to assign their own security group, or another, to a user while the administrator is unavailable.

Emergency Sign-in Privileges

Setup

– File > Security > User Settings > Edit User Security > Emergency Security Groups

Emergency Sign-in Privileges

Reset passwords without System Admin assistance On the Signed In window, click Change Password > Authentication Questions.

Authentication Questions

Setup

• Change your password using authentication questions • Set up questions in your security record

Authentication Questions

Text Shortcuts, also known as macros, allow you to define a keystroke combination that, when entered, automatically expands into an extended amount of text.

– File > Table Setup > Text Shortcuts > ellipsis button > User-defined Field

Creating Text Shortcuts

The Quick Comments table allows you to configure prewritten comments used in a variety of Harvest windows.

– File > Table Setup > Quick Comments

Creating Quick Comments

Using rules in QC files • Ordering Rules: Configure Harvest to order QC

automatically on demand. If a tech places an order on a patient for a sample that hasn’t had QC ordered within a certain timeframe, Harvest orders QC automatically.

• Approval Rules: Control the approval of patient order choices based on the status of the related QC file.

QC Enhancements - Rules

Ordering rule setup – Review Quality Control > open control file > Rules

QC Enhancements - Rules

Approval rule setup – Review Quality Control > open control file > Rules

QC Enhancements - Rules

To establish in-house statistical data, copy an existing QC file and rename it, then collect 20 (or more, depending on the lab) data points. After the data points are collected, use Harvest to calculate the statistical data.

QC Enhancements - Lot Switch

The Lab Tests table defines the parameters for each test for which patient sample and/or QC information will be stored.

– File > Table Setup > Lab Tests

Creating Lab Tests

The Order Choices table is where order choices are constructed using groups of tests or individual tests that can be ordered in Harvest. The order choices are derived from the list of tests in the Lab Tests table.

– File > Table Setup > Order Choices

Creating Order Choices

• Staffing levels – shortage of skilled techs

• Lack of college students entering the field

• Closing of MLT/MT schools

• Lab consolidation – mothership taking smaller “routine” labs

• Economy – decreasing reimbursement

Challenges in Laboratory Medicine

• Reduce the number of hours spent manually reviewing test results

• Standardize result review

• Decrease TAT from sample received to approved

• Focus on problematic samples, such as instrument errors or flags and specimen issues (clot, bubble, short sample)

• Reduce human error

Benefits of Auto-approval

1. Lab Tests table: Set approval low and high for quantitative results.

2. Rules - Result Evaluation table: Set conditions to hold

lab test results that don’t meet auto-approval criteria.

3. Order Choices table: Enable rules-based auto-approval.

Auto-verification in Three Steps

• Hemoglobin A1C is less than Approval Low • Hemoglobin A1C is greater than Approval High • All normal Hemoglobin A1Cs will auto-approve

Hemoglobin A1C

– File > Table Setup > Lab Tests

Approval Low & High

– File > Table Setup > Rules - Result Evaluation

A1C < Approval Low

Actions: Hold Triggered Tests

Glycated Hemoglobin

• Highlight your rule for A1C < Approval Low

• Click Copy (the Copy button is your friend)

• Change condition and description to > Approval High

• Action and associated order choice is the same

Write Rule for A1C > Approval High

– File > Table Setup > Order Choices

Enable Rules-based Auto-approval

Slightly Scary Pop-up

Conditions for Auto-approval • The results are parsed from an

instrument or HL7 interface

• No result evaluation rules are triggered for that order choice

• There are no other rules configured that will prevent the automatic approval

• The order choice-level setting to Enable rules-based auto-approval is selected

Tips & Tricks • Focus on high-volume, lower-

complexity testing, such as chemistry, hematology, and coagulation

• Although Harvest does not automatically approve manually entered results, you can manually input results to test if the rules fire or not

• Rule Usage Report

• Auto-approval Report

• Put your test system to good use

• Validate your rules

• Once validation is complete, move to production system

Test Your Rules

1. Identified and described password reset and general emergency security settings.

2. Configured database tables: Quick Comments, Text Shortcuts, Lab Tests, Order Choices.

3. Discussed enhanced QC file enhancements: Rules and setting up a new lot.

4. Recognized how rules help manage laboratory processes.
