* Employee group life insurance and employee & spousal optional life can be converted to individual plans (no medical if within 31 days)* Your group disability insurance will cease* Your Extended Health Care Package (drugs, travel, dental etc.) can be continued under Plan 2

* If Full Time benefit coverage through the hospital ceases, guaranteed coverage options are available with HCP* If already enrolled in the plan, an automatic transfer to a 65+ Plan is processed, no medical required

Offered exclusively to those applicants already covered for extended health care benefits through a
spouse’s plan or another group plan.
CORE COVERAGE ONLY
CORE BENEFITS UNDER PLAN 1
• LIFE INSURANCE: $10,000 for the Employee
• ACCIDENTAL DEATH, DISEASE AND DISMEMBERMENT: $25,000 for the Employee
• LONG TERM DISABILITY: $ 1,000 monthly benefit for the Employee when unable to work due to illness or injury
LTD AND EIHealth Care Providers’ Long Term Disability benefit works in conjunction with Employment Insurance.
•EI Benefit: 55% of gross annual income up to a specified maximum which is adjusted annually to YMPE
•HCP Benefit: $1,000/month up to a maximum of $5,000 OR 65% of the gross annual salary, whichever is less

Premiums are paid on a monthly basis, by automatic withdrawal from the bank account of the plan member’s choice. Each HCP monthly withdrawal takes place on one of the plan member’s Hospital Pay Days per month.
