Why should I read it?
In Victoria, a worker dies on average every
week. The impact is felt by everyone around
them. Families, loved ones, work colleagues,
employers and mates. The reality is that
almost every one of these deaths could have
been prevented if proper safety procedures
like those outlined in this book were followed.
No-one should die at work.
Who should read it?
If you’re in the construction industry you
should read this book. As you read it, take
a walk around your workplace to look for
potential hazards. Then find ways to fix the
problem in the short term while you work
on long term solutions.

Cause: Falls from heights
SCAFFOLDS • Set up scaffolds on solid, stable ground with loads distributed through base plates and soleplates.
• Platforms to be fully decked and planks secure against uplift.
• Place guardrails, midrails & toeboards on the open sides and ends of all platforms greater than 2m.
• All platforms to have safe access.
• Don’t overload platforms.
• Ensure scaffold is properly braced and tied.
• Scaffolds greater than 4m to be erected, altered and dismantled by certificated scaffolders only.
LADDERS • Do not use ladders where safe alternatives can be used.
• Use step-platforms in preference to ladders.
• Ensure ladders are industrial grade, not domestic grade.
• Do not use 2-handed tools on ladders.
• On step ladders, do not stand higher than second tread from the top.
• Do not stand higher than 900mm from the top of a single or extension ladder.
• Pitch ladder at a slope of 4 (vertical) to 1 (horizontal).
• Ensure ladders are secure.
INCOMPLETE FLOORS • Guardrail open sides, voids and lift shafts.
• Cover penetrations.
ROOFS • Fix safety mesh to metal-deck roofs before roof-laying is started.
• Ensure perimeter protection for all roof work is provided.
• Use roof ladders on very steep-pitched roofs.

Cause: Electrocution
OVERHEAD POWER LINES • Ensure "no go zones" are observed for scaffolds, cranes, EWPs, concrete
booms and earthmoving equipment.
• Use trained spotters where work needs to be done within the "yellow zones."
UNDERGROUND CABLES • Dial before you dig. (1100)
POWERBOARDS • All fuses to be locked.
• RCD to be fitted and tested.
• Ensure powerboards are protected from mechanical damage.
EXTENSION LEADS • Test and tag every 3 months.
• Only use heavy duty leads.
• Protect leads from damage and keep them clear of the ground.
• Do not use piggy-backs or portable domestic power boards.
POWER TOOLS • Use battery-powered tools where possible.
• Test and tag every 3 months.
• Always follow the manufacturer's instructions.

Cause: Structural Collapse
TRENCHES • Shore trenches.
• Use trench shields.
• Batter back the trench sides.
• Keep spoil well clear.
• Keep heavy machinery well clear.
• Ensure a trained person is in charge.
STEEL STRUCTURES • Follow the documented erection sequence strictly.
• Check that weather conditions are safe for erection.
• Maintain structural stability at all stages by using temporary guys, props, braces, etc.
• Only certified riggers to be in charge of trained crews.
BULK EXCAVATIONS • Use physical barriers to separate people from earth walls.
• Only underpin to an engineered plan.
• Ensure a trained person is in charge.
SHORING & FALSEWORK • Do not erect without an engineer’s drawing.
• Do not exceed manufacturer's Working Load Limits.
• Inspect structure against plan prior to loading.
• Load in strict accordance with any instructions specified on the drawing.
TILT-UP & PRE-CAST PANELS • Follow the shop drawings and documented erection sequence strictly.
• Check that weather conditions are safe for erection.
• Do not remove temporary braces without written authority.
• Only certified riggers to be in charge of trained crews.

Cause: Powered Plant
MOBILE PLANT • Use physical barriers to isolate people from operating plant.
• Fit plant with warning devices and reversing beepers.
• Workers who need to be near plant to wear high visibility reflective vests.
MOVING PARTS • Ensure guards are fitted.
OPERATION • All operators to be trained and where appropriate, certificated or qualified.
• Use the right machine for the right task.
• Ensure operator's manual is used.
• Schedule work to avoid operator fatigue.
MAINTENANCE • Test and service as per manufacturer's advice.
• Maintain a logbook.
OVERTURNING • Ensure ground is stable and strong enough for imposed loads.
• Keep clear of excavations and steep inclines.
• Never exceed load charts.
• Check weather conditions are safe for operation.
