INTRODUCTIONForever 21 MasterCard is a collaboration between SM, BDO and MasterCard, This new credit card aims to make shopping easier and enjoyable with the following exciting features and benefits:

• Earn rewards points when you use your card anywhere and use your points to redeem Forever 21 items from the store

Php200.00 = 1 point for all purchases made anywherePhp200.00 = 3 points for all purchases made in Forever 21 branches

• Avail of Zero Percent (0%) installment at 3 and 6 months at any Forever 21 branch (nationwide) and all other BDO merchant stores

• Priority access to sale events and upcoming season offers

• Get special occasion discounts and freebies

• Receive invitations to exclusive Forever 21 gatherings – shopping parties, store events, etc.

• Worldwide acceptance

FOREVER 21 MASTER CARDMEMBER-GET-MEMBER PROGRAMPROGRAM MECHANICS AND TERMS & CONDITIONS

REFERROR:

1.The promotion is exclusively available to the recipients of the SMS and E-direct mailer. These are the existing cardholders of the Forever 21 MasterCard and shall be referred to as the “Referrors”. This consists of F21 accounts approved for year 2011.

2.Promotion period is from June 1 to August 31, 2012.

3. Referrors may refer friends, colleagues and family members provided that their referral has met the indicated criteria. (See Referral below)

4. Only Referrors with referrals whose application forms were received within promo period and subsequently approved shall be part of the promo.

REFERRAL:1.Referral criteria are the following:

• With no principal BDO Credit Card or has not had one in the last 6 months;

• Is 21-40 years old;• Has an annual income of at least P 240,000;• Is residing in the Philippines with a landline;• Has signed and completed the Member-Get-Member (MGM)

application form; • Has submitted the required documents; and• Has met BDO’s review criteria.

2.To find out the status, the referrals can call the customer contact center at (02) 6318000 or domestic toll free number 1800-10-6318000

3.Once approved, the referral should call the customer contact center for card activation.

4.Only the approved referral can receive the Forever 21 MasterCard upon card delivery. He/she MUST present the same ID he/she submitted to BDO during application.

MGM APPLICATION PROCESS:

• Cardholder must first register as a Referror to qualify for the program. To do this, cardholder must first log onto the website: http://f21.bdo-clg.com/forever21. Referror must use his/her existing mobile or email address on file.

• Client must completely fill out the information needed by following the step-by-step process on-line. This should include agreement to the terms and conditions and completion of all required fields for registration.

• Client must then wait for the confirmation of successful registration then may proceed in referring prospective accounts.

• After Referror's successful referral, a notification via SMS/Email is received by the Referral together with the features and benefits as well as promo mechanics.

• Referral must then download the MGM application form from the same site. This form is strictly to be used in order to qualify for the program. The use of forms other than the MGM Application form disqualifies the Referror from earning shopping credits.

• The MGM form must be completely filled out by the Referral.

• The MGM Application Form may be submitted via the same site by attaching the scanned MGM form and required documents and aggreeing to the terms and conditions of the program. Referral may also send the MGM form to any of the following options:

o Send via email to: forever21application@bdo.com.ph and indicate Subject – F21 MGM

o Fax to (02) 6671547 or (02) 6881201o Drop off the form in any of the BDO branch or Forever 21 shops in the

Philippineso Send via mail to

• Required documents for MGM applications are:

a. Photocopy of 1 valid ID. Must be a photo and signature bearing ID card. The acceptable ID cards are the following:

o Passporto Driver's Licenseo Professional Regulations Commission IDo Government Service Insurance System E-Cardo Social Security System Cardholdero Overseas Workers Welfare Administration IDo OFW IDo Seaman's Booko Alien Certificate of Registrationo Government Office ID and GOCC IDo Home Development Mutual Fund IDo Company Ids

* For Foreigners: ACR/ICR. The applicant may present a Special Investor’s Resident Visa & Visa’s Philippine Economic Zone Authority with EO 226.

b. Credit card with other banks.

c. If applicant is not an existing cardholder of any bank, he/she must submit any one (1) of the following:

o ITR of BIR form 2316o Three (3) months payslipo Original Certificate of Employment and ITR or BIR form 2316

d. For Self-Employed, photocopy of latest Income Tax Return (ITR) duly stamped as received by the Bureau of Internal Revenue (BIR), and Audited Financial Statement (AFS).

•Photocopied MGM application forms should still bear the original signature of the Referral. Applicant must sign on all mandatory signature fields.

•Incomplete application form will be called out by the Telesales Team for completion.

•In case of multiple card applications by a single Referral and submitted through different channels:

a. Referror – the Referror of the card application which was first received and subsequently approved by the bank shall be the Referror who will be eligible for the shopping credits.

b. Acquisition Channel – If application received and subsequently approved by the bank of the Referral’s name was received through an acquisition channel apart from the MGM program, then no Referror is eligible for shopping credits.

SHOPPING CREDITS:

1. The Referror will receive P300 shopping credits for every approved Forever 21 MasterCard referral.

2. Shopping credits will be credited in the referror's Forever 21 MasterCard within the end of the following month. Referror will be notified via SMS / email about the shopping credits that will be given within the end of the following month. She/He will be able to view this in the Statement of Account.

3. In case a Referror is found to have made or engaged in invalid, fraudulent or multiple redemptions for the same, duplicate or related applications, as determined by BDO, BDO shall have the right to charge the shopping credits to the Referror's Forever 21 MasterCard.

4. The shopping credits have no expiry date.

HOW TO REFER?

STEP 1 • If you are a new user, register. If you are a registered member, concatenate your email address to Forever21 URL (f21-bdo-clg.com/forever21/killieroar00@yahoo.com )

STEP 2 • Input personal information of your referral.

ADD button enables you to add more referrals.

STEP 3 • Your friend will receive an email stating you referred him to Forever21 together with a link for the application page.

HOW TO APPLY?

STEP 1 • Go to the link provided on email notification (Referral)

STEP 2 • Check the Terms and Conditions to enable the download link of the application form (in PDF file format)

STEP 3 • Sign up the application form. Upload the signed form then submit. • You may also send the application form by clicking the forever21application@bdo.com.ph

STEP 4 • You will receive an email confirming the submitted application form.

3.1.2. User can export to excel(.csv) all records of member’s referrals by stating a specific date range

1.9. MANAGE USERS

This option enables you to add a new user with appointed access level as an administrator or as a user

3.1.4. Edit User

• The user can’t edit the username• The user can’t change the email address• The user can’t change the mobile number

3.1.5. Modify

• The user can modify the access in every module in Admin Panel by checking add, edit or delete checkbox

• Click Save Changes button if the user wants to apply the changes in controlling a module

• Click Cancel button if the user don’t want to save the changes

3.1.6. Search for an existing user

• Enter a specific keyword to search, example above is “NAME”

3.1.7. You can also search by using the following buttons:

• First- shows the first member on the record• Previous- show the previous record of the current account• Next- shows the next record of the current account• Last- shows the last member on the record

2.1. MANAGE CONFIG

This option enables you to add a new CONFIG_NAME and CONFIG_VALUE

Enter a Config name Enter a Config value Then click Save button

3.1.9. Search for an existing config_name and config_value

• Enter a specific keyword to search, example above is “CONFIG_NAME”

2.2. MANAGE TEMPLATESThis option enables you to change the appearance of the template

• Click the status of a specific skin name, whether if the user want to change it to active or set as default.

• Enter a specific keyword to search, example above is “NAME”.
