Addendum No. 1

The following are revisions and/or clarifications to the Bid Documents (Drawings and

Technical Specifications).

A. QUESTIONS

1. Question: Pages 00700-57 & 01600-6 of the specifications mention a one year

warranty. Pages 01030-9 & 01720-4 mention a two year warranty. Please advise

if the contract is to have a one year or two year warranty.

Response: The warranty period for the work shall be one-year from date of

Substantial Completion. See Section C of this addendum for revisions to Sections

01030 & 01720.

2. Question: For the installation of the new below grade structures, is a caisson method acceptable? Response: Caisson method shall not be an acceptable method of installing below grade structures.

3. Question: Can a line item be added to the bid form for compensation if sheeting

is to be left in place? (Reference page 02221-10)

Response: The base project bid shall be based on the removal of all sheeting used to facilitate construction, therefore the addition of a bid item for sheeting left in place will not be added to the bid form. If the Contractor is directed to leave any sheeting in place, this will be considered a change to the Contract and the Contractor will be compensated in accordance with the Contract Documents.

4. Question: Can the geotechnical report be posted in an addendum? (Reference

Page 00800-1)

Response: The geotechnical report and FDEP permit are being added as attachments to the Specifications. See Section C of this addendum for revision.

C. TECHNICAL SPECIFICATIONS

1. CHANGE the paragraph in Specification Section 01030, Part 1.21, A to read as

follows:

“A. It is assumed that portions of the work will be completed prior to completion

of the entire work. Upon completion of construction in each individual

facility, including testing, if the Owner, at his sole discretion, desires to

accept the individual facility, the Contractor will be issued a dated certificate

of completion and acceptance for each individual facility. The Owner will

assume ownership and begin operation of the individual facility on that date

and the one-year guaranty period shall commence on that date. The Owner

has the option of not accepting any individual completed facility, but

accepting the entire work as a whole when it is completed and tested.”

2. CHANGE the paragraph in Specification Section 01720, Part 1.07 A to read as

follows:

“A. This manual shall contain all warranties, guarantees, bonds, certificates,

and similar documents, including those customarily provided by

manufacturers and suppliers which cover a period greater than the one-year

correction period. Show Owner as beneficiary on these documents.”

3. ADD the following paragraphs to Section 02222, Part 3.03, A.

“2. In general, sheeting and bracing shall be removed as the excavation is

backfilled in such a manner as to avoid the caving in of the bank or

disturbance of adjacent areas or structures. The voids left by withdrawal of

the sheeting and bracing shall be carefully filled by jetting, ramming or other

means approved by the Engineer. Permission shall be obtained from the

Engineer prior to removal of any sheeting or bracing. Permission shall not

relieve the Contractor of any responsibility for damage due to failure to leave

such sheeting and bracing in place.”

3. The Engineer may order, in writing, any or all sheeting or bracing to be left

in place for the purpose of preventing injury to adjacent structures, property,

etc. If left in place, such sheeting shall be cut off at the elevation ordered,

but in no case less than thirty-six (36) inches below the existing grade.

Bracing remaining in place shall be driven in tight. The right of the Engineer

to order sheeting and bracing to remain in place shall not be construed as

creating any obligation on his part to issue such orders. Payment for

sheeting and bracing, unless specifically called for on the Drawings shall

not be paid under separate item, but shall be included in the payment for

other items of Work.”

4. DELETE paragraph in Section 02222, Part 3.07, A.

5. ADD the following to the end of the Table of Contents.

“ATTACHMENTS

Attachment 1 - Geotechnical Report

Attachment 2 – FDEP General Permit for Constructing a Domestic Wastewater

Collection/Transmission System”

6. ADD the attached Geotechnical Report and FDEP Permit to the end of the

Technical Specifications.

NOTIFICATION OF ACCEPTANCE OF USE OF A GENERAL PERMIT

Dear Mr. Dickson ,

This letter acknowledges receipt of your Notification/Application for Constructing a Domestic

Wastewater Collection/Transmission System for the subject project. Our Office received the

Notice on February 14, 2020.

This is to advise you that the Department does not object to your use of this general permit for the

following: twelve-inch diameter gravity pipe(s), with one manhole, and one lift station with

eight-inch diameter force mains pipe(s).

Please note the attached requirements apply to your use of this general permit for constructing the

proposed domestic wastewater collection/transmission system.

You are further advised that the construction activity must conform to the description contained

in your Notification/Application for Constructing a Domestic Wastewater

Collection/Transmission System and that any deviation may subject the permittee to enforcement

action and possible penalties.

REQUIREMENTS FOR USE OF THE GENERAL PERMIT FOR DOMESTIC

WASTEWATER COLLECTION/TRANSMISSION SYSTEMS:

1. This general permit is subject to the general permit conditions of Rule 62-4.540, F.A.C., as

applicable. This rule is available at the Department’s Internet site at:

http://www.dep.state.fl.us/water/rulesprog.htm#ww [62-4.540]

2. This general permit does not relieve the permittee of the responsibility for obtaining a dredge

and fill permit where it is required. [62-604.600(6)(b)1]

3. This general permit cannot be revised, except to transfer the permit. [62-604.600(6)(b)2]

4. This general permit will expire five years from the date of issuance. If the project has been

started and not completed by that time, a new permit must be obtained before the expiration

date in order to continue work on the project. [62-4.030]

5. Upon completion of construction of the collection/transmission system project, and before

placing the facilities into operation for any purpose other than testing for leaks or testing

equipment operation, the permittee shall submit to the Department’s Southwest District Office

Form 62-604.300(8)(b), Request for Approval to Place a Domestic Wastewater

Collection/Transmission System into Operation. This form is available at the Department’s

Internet site at: http://www.dep.state.fl.us/water/wastewater/forms.htm [62-604.700(2)]

6. The new or modified collection/transmission facilities shall not be placed into service until the

Department clears the project for use. [62-604.700(3)]

7. Abnormal events shall be reported to the Department’s Southwest District Office in accordance

with Rule 62-604.550, F.A.C. For unauthorized spills of wastewater in excess of 1000 gallons

per incident, or where information indicates that public health or the environment may be

endangered, oral reports shall be provided to the STATE WATCH OFFICE TOLL FREE

NUMBER (800) 320-0519 as soon as practical, but no later than 24 hours from the time the

permittee or other designee becomes aware of the circumstances. Unauthorized releases or

spills less than 1000 gallons per incident are to be reported orally to the Department’s

Southwest District Office within 24 hours from the time the permittee, or other designee

becomes aware of the circumstances. [62-604.550]
