ENR 1.14 AIR TRAFFIC INCIDENTS
1. Reporting Incident or Occurrences
1.1 Article 86 and Regulation 16 of Schedule 15 of the Air Navigation (Hong Kong) Order
1995 details the requirements for Mandatory Reporting. Guidance and information on
Civil Aviation Department reporting procedures are given in document CAD 382, The
Mandatory Occurrence Reporting (MOR) Scheme.
1.2 In addition to the MOR Scheme, under Regulation 5 of the Hong Kong Civil Aviation
Regulation (Investigation of Accident) [CAP448B], the Chief Inspector of Accident of
the Civil Aviation Department shall be given notice of an aircraft accident or incident
under the established Mandatory Incident Reporting (MIR) System and the Voluntary
Incident Reporting (VIR) System. The policies and procedure on the Incident
Reporting System are published in Accident Investigation Bulletin No.1/09 in the CAD
website [ http://www.cad.gov.hk/ ].
2. Definitions
2.1 AIRCRAFT PROXIMITY (AIRPROX)
A situation in which, in the opinion of a pilot, the distance between aircraft as well as
their relative positions and speed have been such that the safety of the aircraft
involved may have been compromised.
2.2 REPORTABLE OCCURRENCE
An occurrence other than an AIRPROX which has endangered, or if not corrected
would have endangered an aircraft, its occupants or any other person.
2.3 ACCIDENT
An occurrence associated with the operation of an aircraft which takes place between the time any person boards the aircraft with the intention of flight until such time as all such persons have disembarked, in which:
a person is fatally or seriously injured as a result of:
a) being in the aircraft , or
b) direct contact with any part of the aircraft, including parts which have become detached from the aircraft, or
c) direct exposure to jet blast,
except when such injuries are from natural causes, self inflicted or inflicted by other persons, or when the injuries are to stowaways hiding outside the areas normally available to passengers and crew.
2.4 SERIOUS INCIDENT An incident involving circumstances indicating that an accident nearly occurred.

2.5 INCIDENT
An occurrence, other than an accident, associated with the operation of an aircraft which affects or could affect the safety of operations.
3. Report Forms
3.1 All the applicable Reporting Forms are available for download from the CAD website
[ http://www.cad.gov.hk/english/applications.html ]
3.2 Sample reporting forms are also attached at the end of this Chapter for reference.
4. Reporting under MOR System for AIRPROX (Form DCA 232)
4.1 An AIRPROX report should be made whenever a pilot considers that the safety of the
aircraft may have been compromised by the proximity of another aircraft during flight
within Hong Kong airspace. An initial report of the AIRPROX incident should be made
by radio to the ATS Unit which the aircraft is in communication at the time. If this is
not done at once, it may be impossible to trace the reported aircraft.
4.2 The initial report must be prefixed with the word 'AIRPROX', and should be in the
following form:
a) AIRPROX report; b) R/T callsign; c) Position at time of AIRPROX (heading or route and true airspeed); d) Flight level, altitude or height (climbing, descending, turning or level flight); e) Flight weather conditions; f) Time of AIRPROX; g) Description of other aircraft; and h) Description of the AIRPROX including flight paths, vertical and horizontal
sighting and miss distances, and avoiding action taken.
4.3 The ATS unit receiving the report may, where necessary, ask for any additional
information.
4.4 If it is not possible to report the AIRPROX by radio, a report should be made to the Air
Traffic Services Watch Manager immediately after landing:
a) in Hong Kong by telephone to +852 2910 6821;
b) if overseas : i) by fax to +852 2910 1178; ii) by e-mail to [ atmdwsu@cad.gov.hk ]; iii) by AFTN to VHHKZQZX or VHHHZRZX.
4.5 Any AIRPROX report made to air traffic services, whether by radio, telephone, fax, e-
mail or AFTN, must be confirmed within seven days by completing an AIRPROX
Report (DCA Form 232).
4.6 To minimise delay in reporting an incident, the confirmatory report shall be submitted
to the Assistant Director-General of Civil Aviation (Flight Standards):

4.7 Some operators may require to submit confirmatory reports through their company or
agency. This is acceptable, provided the AIRPROX Report, signed by either the pilot
or a responsible official of the company or agency, is forwarded in accordance with
paras 4.6 above.
4.8 In the absence of exceptional circumstances, follow-up action on radio or telephone
reports will cease after seven days, unless the confirmatory report as detailed in para
4.5 has been received.
4.9 A pilot leaving Hong Kong for more than seven days must transmit a confirmatory report to his company or agency in Hong Kong to ensure action is continued on the reported AIRPROX. On receipt of such a message, the company or agency should complete the necessary AIRPROX Report and submit it to the Assistant Director-General of Civil Aviation (Flight Standards) in accordance with para 4.6 above without delay.
4.10 Pilots should co-operate by ensuring that the fullest possible information is given in
every case. Company or agency officers concerned are asked to facilitate action on
reports by prompt handling in strict accordance with the procedures.
4.11 AIRPROX Reports will receive immediate action and operators/pilots will be advised of the Civil Aviation Department’s findings. They are requested not to seek further information on the AIRPROX Report from ATS Units, any requests for information should be addressed to the Director-General of Civil Aviation.
5. Reporting under MOR system for Reportable Occurrence (Form DCA 201)
5.1 In addition to the mandatory reporting requirement, voluntary reporting using this Form DCA 201 is also encouraged.
5.2 The incident shall be reported within 96 hours of the event.
5.3 To minimise delay in reporting an incident, the report may be submitted to the

6. Reporting of Runway Incursion, Bird Strike or Wind Shear and Turbulence
6.1 Due to the specialized nature of the information required on these occurrences,
special forms are provided for recording information.
6.2 Runway Incursion Reporting Form (DCA 235) (also used for reporting of obstructions
on the runway) is available from the CAD website:

7. Reporting under MIR System for Accident or Serious Incident - Aircraft
Accident / Incident Reporting Form (Form DCA 233)
7.1 When there is an aircraft reportable accident or serious incident, the commander and/or operator, maintenance service provider or ATC controller shall report to the Accident Investigation Division of CAD using Form DCA 233.

7.3 The report can be sent to the duty ATS Watch Manager by:

8.1 For occurrences that are not captured by the MIR System, all parties are encouraged to report such incidents to AID of CAD using Form DCA 234.

9. Other reportings under ICAO initiative for data collection and analysis purposes
9.1 Wake Vortex
9.1.1 Pilots should report any wake vortex occurrence to the regulatory office of CAD by
completing the Wake Vortex Encounter Reporting Form (appended to this Section).
The completed form will be forwarded to ICAO for necessary action.
9.1.2 Alternatively, pilots can submit the report online direct to ICAO.
9.1.3 Contact fax, email, address and website URL information are contained in the
Reporting Form.
