Plan • Development of care bundles (45) for all care

pathways: • Pre-admission information sheet – for the patient and family • Discharge information sheet – for the patient and family • Cue Cards for the nursing staff – these are evidence based daily care

plans for standardising patient care. They are laminated and sit in the patient folder by the bed

• Education – in the form of power point presentations on each computer in the ward for quick referencing for all staff re each diagnosis
