Does Medicare Part A pay for a skilled nursing facility?By Vicki Wilson
I receive telephone calls and personal visits every day from people requiring the services of a SNF (skilled nursing facility). Usually, their parent or spouse is in a hospital, nearing the end of their stay there, and it has been determined that the patient would benefit from continued rehabilitation before returning home. My main focus here at Briar Hill Health Care Residence is to ensure a smooth transition for the patient from the hospital, or home, to Briar Hill. I work with the Director of Nursing to first ensure we can meet all of the medical needs of the patient. I then verify that we have an opening in our short-term rehabilitation wing, or elsewhere in the facility if that room would better serve the patient. I stay in constant communication with the social worker or case manager at the hospital to ensure a seamless transfer of services to that patient. As you leave the hospital to travel to Briar Hill, you can be assured that we are ready to welcome you and guide you on your path to further recovery.
Skilled nursing care is a level of care that requires the daily involvement of skilled nursing or rehabilitation staff.
Examples of this type of care include IV injections or fluids, wound care or physical, occupational or speech therapy.
Custodial care (long term care) is care for the patient such as help with activities of daily living. Activities of daily living include bathing, dressing, eating, moving around, getting in and out of bed, and using the bathroom. The need for custodial care cannot qualify you for Medicare coverage in a SNF if that is the only kind of care you need. However, if you qualify for coverage based on your need for skilled nursing care and rehabilitation, Medicare will cover all of your care needs in the facility, including help with activities of daily living. Medicare does not cover long-term care only in the nursing home setting.
For Medicare to pay for your stay at a SNF, you must meet certain federal guidelines. You must have a three-day minimum medically-necessary inpatient hospital stay for an illness or injury related to your need for further rehabilitation. Your physician must certify and give written orders that you require daily skilled nursing care. Medicare Part A will pay for a stay that includes a semi-private room, meals,
skilled nursing and rehabilitative services and certain other services and supplies for up to 100 days in a benefit period.
A benefit period is the way that Medicare measures your use of SNF services. A benefit period begins the day you are admitted to the SNF. The benefit period ends when you haven’t received any inpatient hospital care or skilled care in a SNF for 60 days in a row. If you go into a hospital or SNF after one benefit period has ended, a new benefit period begins.
Clear as mud? Now let’s talk about dollar figures. As I mentioned earlier, you have up to 100 days in a benefit period. The first 20 days in a SNF are paid 100% by Medicare Part A. Beginning on Day 21, you have a copay in the amount of $141.50 per day. That rate in 2012 will probably rise to $144.50 per day. Hopefully, you have a secondary insurance that will pick up all or most of that amount. Many people have secondary insurance supplied by a prior employer or they choose to pay privately for additional coverage. You must pay all costs for each day after Day 100 in a benefit period.

If you’re unhappy with the way things are, you’ll want to read this ...
By Dr. Tad Roediger, Chiropractor
Americans are far more educated and aware then they used to be, and that’s causing a profound change in the way we view things. Let me explain. Imagine driving your car, and the dashboard oil light comes on. Would you cover the light up with tape, or would you fix the problem? Of course you wouldn’t just cover it up, you know better! But isn’t that what we do with our bodies when we seek treatment for the symptoms and not treat the source of the problem?
Let me tell you my story. Back when I was a sophomore in college trying to make the football team, I developed a painful low back condition known as “sciatica”. In my case it came on following an injury. The pain in my back and leg was so intense that I couldn’t walk easily, sit still, sleep, and sometimes I couldn’t even concentrate or think straight. I was afraid that I’d flunk out of college if the disability continued. After considering surgery (that was the only option, according to the surgeon) I decided to have surgery.
But there is more. After surgery, I continued to have almost the same amount of pain in my back and leg. A friend of mine convinced me to give his Chiropractor a try. The Chiropractor took me in right away and did some tests. He told me the tests showed that I had several misalignments in my spine that were putting pressure and tension on my nerve system (even after the surgery) and that it was serious. He was confident that he could help me with a series of adjustments that would alleviate the pressure and allow my
body to heal itself. He had me get on a special table then “adjusts” my spine. The adjustment didn’t hurt, it actually felt good. I still had some pain but I
could walk, sit and move better. After more adjustments, I noticed I kept improving and even slept better.
It worked so well that I went to chiropractic school myself. But, tell me, what if I just did nothing following the surgery, where would I be today?
It’s strange how life is. Now people
come to see me with their low back and sciatic problems. They also come to me with their headaches, migraines, chronic pain, neck pain, shoulder /arm pain, numbness in limbs, whiplash from car accidents, backaches, ear infections, asthma, allergies, sports injuries, just to name a few.
My wife Sharon, on the right in the photo holding our son, Ty, suffered for years with migraine headaches. She took ibuprofen everyday. We found her migraines were from misaligned vertebrae in her neck, we adjusted them, now she rarely has migraines. Courtney is our teenager and runs long distance races. She gets treated due to the stress of training. Ty, our youngest, gets checked weekly to make sure his growing spine and body are working at its best.
WhaT seTs me apaRT ... in the chiropractic field is my use of the activator methods Chiropractic Technique (amCT); a treatment system that utilizes a small hand-held instrument that applies a quick, low-force, gentle chiropractic treatment directly to the source of your pain to alleviate nerve pressure to allow the body to heal itself.
here’s what some of my patients had to say: “A pinched nerve in my back sent me to see Dr. Roediger. With each adjustment the pain decreased and I was feeling myself again. I am always able to get in and out so my driver is not waiting long”. – Lucinda s., middlefield
“I had been suffering with back pain for 15+ years, even resorted to spinal blocks and back surgery. Within the first week of treatment, I was feeling really good. Dr. Roediger and his staff are incredibly warm and friendly.” – Judy C., Chardon
“After 3 1 / 2 years of unhelpful pain management for low back pain, a good friend sent me to Dr. Roediger... His treatment has made such a difference in my life in such a short amount of time.” – Vern F., Chardon

This auto insurance is designed exclusively for AARP members – and is now available through your local agent!
‡ The AARP Automobile Insurance Program from The Hartford is underwritten by Hart ford Fire Insurance Company and its affiliates, One Hartford Plaza, Hartford CT 06155. In Washington, the Program is underwritten by Trumbull Insurance Company. AARP membership is required for Program eligibility in most states. This Program is provided by The Hartford, not AARP or its affiliates. The Hartford pays a royalty fee to AARP for the use of AARP’s intellectual property. These fees are used for the general purposes of AARP. Applicants are individually underwritten and some may not qualify. Specific features, credits and discounts may vary and may not be available in all states in accordance with state filings and applicable law. The premiums quoted by an authorized agent for an AARP program policy include the costs associated with the advice and counsel that your local agent provides.‡ Some benefits, including First Accident Forgiveness and the Disappearing Deductible, are only available with the optional Advantage Plus package. A policy without these benefits is also available [Call for details.] To qualify for these two benefits, all drivers on the policy must have a clean record (no accidents or violations) for five consecutive years in most states. For the Disappearing Deductible, these five years must include a period of three consecutive years as a policyholder in the AARP Auto Insurance Program (commencing after the effective date of the policy issued through this offer). PA drivers are not eligible for the complete disappearance of the deductible, although it will be reduced to a minimum of $100. The First Accident Forgiveness benefit is not available in Delaware.† If you are age 50 or older, once you’re insured through this Program for at least 60 days, you cannot be refused renewal as long as applicable premiums are paid with due. Also, you and other customary drivers of your vehicles must retain valid licenses, remain physically and mentally capable of operating an automobile, have no convictions for driving while intoxicated and must not have obtained your policy through material misrepresentation. 107292 Rev

Attention Arthritis Suffers: The Activator Chiropractic Method Can Help!
The term “arthritis” is used to describe a variety of joint conditions. Each of the conditions has a different cause. Rheumatoid and psoriatic arthritis are caused by deficiencies in the body’s autoimmune system. The most common form of arthritis, osteoarthritis, is caused by degenerative joint diseases that occur as the result of joint trauma, infection, or the aging process. Each form of arthritis can cause pain, limit mobility, and keep you from doing the things you love. Even common activities like sitting at your desk or grocery shopping can become painful ordeals. Unfortunately, millions of Americans are afflicted by some form of arthritis – it is the leading cause of disability in people over the age of 55. Many arthritis sufferers are told they must simply live with their pain and discomfort. Pain medications become a daily routine. But the truth is; arthritis sufferers don’t have to let pain control their lives. For over a century, Chiropractic has helped the arthritic combat their pain by naturally restoring the body’s harmony. But don’t
think of Chiropractic as simply an arthritis treatment. Think of it as a health care system that restores the body’s natural alignment and functions and improves your overall health by allowing your body to heal itself naturally. Experienced, caring practitioners of Chiropractic like Dr. Tad Roediger of ROEDIGER CHIROPRACTIC can help you improve the quality of your life and relieve your physical pain by realigning your body so the healing process can begin. Dr. Roediger knows what it’s like to live with constant pain. After suffering an injury while playing football in college, Dr. Roediger developed a painful condition known as sciatica. His pain made it hard for him to do basic daily functions like walking or sitting – it even disrupted his sleep. For a long time he could not find relief – surgery did not fully correct the problem. Then a friend recommended Chiropractic treatment. Chiropractic helped Dr. Roediger take his life back. Now Dr. Roediger is the one helping others overcome the pain that controls their lives. In the past some patients and even some doctors have expressed concern over
the use of Chiropractic on arthritic joints. They argue that Chiropractic treatments involving strenuous manual adjustments, extreme twists and turns, can potentially aggravate a patient’s already aching joints. That’s why Dr. Roediger uses the innovative Activator Methods Chiropractic Technique (AMCT). AMCT utilizes the latest advances in orthopedic, neurological, and chiropractic technology to deliver safe, consistent, low-force chiropractic adjustments without any awkward twists or turns. Why live with arthritic pain? Chiropractic treatment is very affordable and highly effective. Take the first step towards relief and call for an appointment today! ROEDIGER CHIROPRACTIC is located at 401 South St., Bldg. 2A, Village Station in Chardon across from Bass Lake Restaurant. Office hours are 10am to 6pm Monday through Friday. Appointments are recommended and walk-ins are welcome. Call today at 440-285-0756 to schedule an appointment. You may also visit ROEDIGER CHIROPRACTIC on the Web at www.roedigerchiropractic.com.

Medicare continued from page 2I’ve only touched on the original Medicare plan run by the
Federal government. There are also Medicare Advantage Plans, like an HMO or PPO, available. These plans are run by private companies and are approved by Medicare. Costs vary by plan and may be either higher or lower than we’ve discussed. You must check with your individual plan to verify the associated fees.
All of this can be terribly confusing, especially at a time that you are also coping with the illness or injury that put you into this position. It is important to know that the hospital social worker and I are here to assist you. We do this every day. We will let you know what coverage is available to you so that you can make informed decisions about your choices for continued care. Please call me at any time that you have questions. My number is 440-632-5241. If I am unable to answer your individual questions, I should be able to steer you to someone that can.
This information is available to you in detail in the Medicare & You 2012 booklet, or you can go to the Medicare website at www.medicare.gov.
Vicki Wilson is the director of admissions/marketing at Briar Hill Health Care Residence, 15950 Pierce St., P.O. Box 277, Middlefield, 440-632-5241.
