Physicals must be on a Certificate of Child Health Examination form with a Revision date at the top of form of 2013 or after.

• IHSA forms WILL NOT be accepted.

Physical dates must be within 365 days (1 year – August 13, 2017) prior to the first day of school for incoming freshman year.

Do not wait to make appointments! Students can be excluded from school.
