A. Add the following notes: 1. Provide and install a copper termite shield and sill sealer between the concrete foundation
and wood sill plate continuous around the entire perimeter of the exterior foundation wall. a. The sill sealer is to be 5 ½” x ¼” thick Styrofoam Sill Seal foam gasket
manufactured by Dow (or equal). b. The termite shield is to be a 16 oz. thick copper sheet continuous. Lap all seams
6” and exterior edge of the sheet is to extend 1” beyond face of siding. 2. The soil around the entire perimeter of the building is to be treated with Termidor
termiticide manufactured by BASF or Premise by Bayer. Apply in accordance to manufacturer’s instructions by qualified applicators.
3. The bottom edge of the water tables shall be set at ¾” below the bottom edge of the wall sheathing at all points.

2. ALLOWANCES

Funds will be drawn from allowances only by change order. At the closeout of Contract, funds remaining in allowances will be credited to Owner by Change Order. For complete description of allowances see Section 01210.
3. ALTERNATES
Alternate bid prices shall contain all charges for overhead, profit, insurance, allowance for waste and the sum quoted shall represent the work complete and in place to the Owner. For complete description of alternates, see Section 01220 along with related technical sections and drawings. 8. BONDS & ACCOMPANYING SUBMITTALS
Performance, Labor and Material Bonds (Assurance of Completion) in an amount equal to 100% of the contract sum are required by the OWNER. They shall be executed in such forms, and with such securities that is acceptable to the OWNER, and be placed with a Company also acceptable by the OWNER. Required Performance, Labor and Material Bond covering the Contract, such will be available from the Surety Company listed below. Cost of bonds are included in the Base bid amount. 9. OVERHEAD & PROFIT - ADDITIONAL WORK
BIDDER agrees payment for overhead and profit on extra work (See General Conditions) combined shall not exceed the following percentage of cost basis. To the Contractor for Work performed by his own forces maximum 10 percent of the cost. To Subcontractors for Work performed his own forces maximum 10 percent of the cost. To the Contractor or Subcontractor for Work performed by his subcontractors maximum of 5 percent of the cost, not including subcontractor or sub-subcontractor's overhead and profit. Overhead and profit is to include the Contractor’s Project Manager, all administrative expenses and personnel, change estimate preparation, mobilization, setup & break-down, meetings, all safety related costs, cleanup costs and storage costs pertaining to the changes in the work. Site supervision may only be included if an extension to contract time is approved in the change proposal.
10. ADDITIONAL PROPOSAL PROVISIONS
If written notice of the acceptance of this BID is mailed, telegraphed or delivered to BIDDER within 60 days after the opening of the bids, or any time thereafter before this BID is withdrawn, BIDDER, will within 10 days after such notice, execute and deliver an agreement in the form specified.

2. ALLOWANCES
The Contractor shall include the cost of the following allowances in his Bid Price. Signage: $ 3,000.00 Gazebo: $ 15,000.00 (Included only in Alternate No. 1) Security: $ 3,000.00 Entrance Awning: $ 4,000.00
Funds will be drawn from allowances only by change order. At the closeout of Contract, funds remaining in allowances will be credited to Owner by Change Order. For complete description of allowances see Section 01210.

The following Unit Prices, if accepted in the award of this Contract, shall be used in establishing the adjustment of Contract Price for additions to or deductions from the Work in accordance with the applicable section of the General Conditions and the Supplementary General Conditions. Unit Prices listed shall include all costs, profit and overhead and no further surcharges are to be added to any Unit Price item of Work that may be ordered done. Work omitted from the contract will be calculated at 100 percent of the additional work unit prices.

8. BONDS & ACCOMPANYING SUBMITTALS
Performance, Labor and Material Bonds (Assurance of Completion) in an amount equal to 100% of the contract sum are required by the OWNER. They shall be executed in such forms, and with such securities that is acceptable to the OWNER, and be placed with a Company also acceptable by the OWNER. Required Performance, Labor and Material Bond covering the Contract, such will be available from the Surety Company listed below. Cost of bonds are included in the Base bid amount. BIDDER submits for approval as his surety:

Form 2530 has been fully completed and indicates all previous involvement or non-involvement with H.U.D. Multifamily Housing Projects, either as a principal of the firm or as a member of a previous company. IT IS MANDATORY THAT THIS FORM BE FILLED OUT COMPLETELY AND SUBMITTED WITH BID DOCUMENTS. REPRESENTATIONS, CONDITIONS AND OTHER STATEMENTS OF BIDDERS: As a condition of bidding, the Bidder by submitting and signing this proposal, certifies that the attached H.U.D. Form 5369-A has been fully completed and is attached to the BID. IT IS MANDATORY THAT THIS FORM BE FILLED OUT COMPLETELY AND SUBMITTED WITH BID DOCUMENTS. FAILURE TO COMPLY MAY CONSTITUTE REJECTION OF BID DOCUMENTS.
9. OVERHEAD & PROFIT - ADDITIONAL WORK
BIDDER agrees payment for overhead and profit on extra work (See General Conditions) combined shall not exceed the following percentage of cost basis. To the Contractor for Work performed by his own forces maximum 10 percent of the cost. To Subcontractors for Work performed his own forces maximum 10 percent of the cost. To the Contractor or Subcontractor for Work performed by his subcontractors maximum of 5 percent of the cost, not including subcontractor or sub-subcontractor's overhead and profit. Overhead and profit is to include the Contractor’s Project Manager, all administrative expenses and personnel, change estimate preparation, mobilization, setup & break-down, meetings, all safety related costs, cleanup costs and storage costs pertaining to the changes in the work. Site supervision may only be included if an extension to contract time is approved in the change proposal.
10. ADDITIONAL PROPOSAL PROVISIONS
If written notice of the acceptance of this BID is mailed, telegraphed or delivered to BIDDER within 60 days after the opening of the bids, or any time thereafter before this BID is withdrawn, BIDDER, will within 10 days after such notice, execute and deliver an agreement in the form specified.
