New digital technologies have been enthusiastically embraced by the profession. This is especially so in restorative techniques with CAD/CAMdesign and manufacture of restorations. In addition, new materials and protocols have facilitated “minimal intervention” concepts in toothrestoration. But has embracing these advances resulted in less prudentbiomechanical considerations in restoration of the endodonticallytreated tooth? Are the concepts of ferrule and resistance form stillimportant? Is “out with the old and in with the new” always appropriate?

The traditional goals of endodontic treatment have focused on the“prevention and treatment of apical periodontitis”. Recent philosophiessuggest that a change in focus to “the retention of asymptomatic andfunctional teeth” is a more appropriate patient focused goal. This lecture will look at how this change in focus has in�uenced the waythat endodontic treatment is carried out, the critical role that newtechnologies play in this evolution and challenges encountered withthis approach.
