Simple bone cyst (SBC) is a benign osseous lesion mostcommonly affecting the mandible. The lesions are intraosseousasymptomatic pseudocysts and are usually seen during routineradiographic examination. Young woman, 17, with no extraoralswellings was referred to our stomatology service after an or-thodontic evaluation to assess an intraosseous lesion in themandible. Panoramic radiograph showed a well-defined radiolu-cent image with undulating borders that extended from the rightfirst molar to the right lateral incisor in the mandibular bone.Clinicoradiographic features suggested a diagnosis of SBC.Incisional biopsy was done; curettage of the bony cavity was thedefinitive treatment. Histopathological analysis revealed a bonewall cavity with no lining epithelium. After a 2-year follow-up,the patient is currently disease-free. This case stresses theimportance of a detailed routine dental examination.

Woman, 78, who formerly smoked was diagnosed withlocally advanced squamous cell carcinoma (SCC) of the base ofthe tongue. The patient underwent radiotherapy, then 6 monthsafter radiation therapy was complete, in a periodic follow-upconsultation with the Dental Oncology Service, two previouslyunnoticed asymptomatic and fibroelastic submucosal noduleswere seen in the anterior left lateral border of the tongue. Basedon the clinical and imaging features, an incisional biopsy wasperformed, leading to a diagnosis of undifferentiated SCC. Thepatient underwent partial glossectomy and neck dissection. OralSCC recurrence may present clinically in unusual ways. The casedemonstrates that complete, accurate history and examinations,systematic periodical clinical evaluation of the oral cavity, andappropriate imaging exams are essential. This case highlights theimportance of periodical dental evaluation and multidisciplinaryteams in cancer treatment.

Histoplasmosis, a systemic fungal disease, affects primarilythe lungs. Oral cavity manifestations can be misdiagnosed asother ulcerated lesions, such as squamous cell carcinoma, tuber-culosis, paracoccidioidomycosis, and other fungal lesions.Woman, 88, was referred to the Stomatology Department toevaluate a painful ulcer in her mouth of 2 months’ duration.During the history and examination, the patient did not revealimmunosuppressive diseases. On clinical examination she had anulcerated lesion measuring about 5 cm whose surface had a

mulberry-like granulomatous aspect on the lower alveolar ridge.The preliminary diagnoses were paracoccidioidomycosis or otherfungal infection. An incisional biopsy was performed with spe-cific staining (PAS and G. Grocott) and suggested histoplasmosis.Fungal culture was also performed, confirming Histoplasmacapsulatum infection. The patient was referred to a center forinfectious diseases for treatment. In most reported cases, histo-plasmosis is associated with immunosuppression, unlike thepresent case.

PE-046 - ATYPICAL OSTEOMYELITIS OF THE JAW.LUHANA GEDOZ, SABRINA POZATTI MOURE, RUBEMBERALDO DOS SANTOS. ULBRA-CACHOEIRA DO SUL.

Man, 43, a cocaine user for 20 years, presented with symp-toms of edema, purulent drainage, and exposure of the bonytissue in the left alveolar lingual area. Imaging did not detect toothinfection. The patient reported that he had a habit of rubbingcocaine on his gums. Incisional biopsy was done, along with theremoval of bone debris; the patient was prescribed an antibiotic.Histopathological examination showed granulation tissue andbone sequestration. The diagnosis was jaw osteomyelitis withbone sequestration caused by the friction of cocaine use. Thiscase had a traumatic etiology, specifically, rubbing cocaine on thegums, which injured the epithelial tissue and allowed the pene-tration of bacteria, leading to the infection. Removal of the bonedebris accompanied by discontinuation of the habit and antibiotictherapy are effective for treating this type of lesion.

Herpes is a viral disease caused by a recurrent infection withsimplex herpes virus (HSV) of the family of human herpesviruses(HHV). In some countries, like the United States, the prevalenceof infected individuals is as high as 90% of the entire population.Patient, 30, reported a history of pain, burning, itching, and theformation of papules, followed by vesicles on the lower lip. Thisproduced a sharp increase in volume of the lower lip associatedwith purulent secretion that suggested secondary infection of theherpetic lesions and crusting. In most cases of HSV, clinicalexamination allows a correct diagnosis. This case was compli-cated by exacerbation of the typical signs and symptoms, whichcan make diagnosis difficult.

The basal cell skin cancer is the most common skin lesion inadult Caucasian patients. It originates from the basal layer of theskin; it is a primary malignancy and spreads slowly.
