Placing a conductive object too closeto a high-amp source of current
Sparks (from dropping tools, rackingbreakers, replacing fuses, etc.)
Equipment failure due to the use ofsubstandard components, improperinstallation, or just normal wear and tear!
Circuit breaker failure dueto lack of maintenance oroveruse of the reset function
Breaks or gaps in insulation
Dust, corrosion and otherimpurities on the surfaceof the conductor
What are common causes of arc flash?
Removing or installing circuitbreakers and fuses
Working on control circuits
Cleaning starter buckets in an MCC
Taking a voltage reading
Applying safety grounds
Throwing a circuit breaker
Operating disconnect switches
Installing conduits andbus duct switches
Tasks that may expose you to danger:
Whenever possible, lockout equipmentbefore service and maintenance.
Conduct a hazard assessment andfollow approved safe work procedures.
Wear appropriate protective clothingand equipment (see chart).
Use insulated gloves, tools, blankets and barriers.
Keep unqualified personnel outsideof the flash protection boundary.
Keep equipment clean and freefrom dirt and corrosion.
Avoid operating equipment athigher than the rated voltage.
Be careful to avoidgenerating sparks.
