1) Spanish Chronic Disease Self-Management Program

Programs are licensed and tested by the Stanford University Patient Education Research Center

• Adults 18 and over with chronic diseases (including: arthritis, diabetes, heart disease, and cancer.)

• Taught by 2 trained leaders, professionals, or lay leaders who have a chronic disease themselves

• Classes in community locations • Run for 2.5 hrs once weekly• Classes are grouped by disease• Highly interactive to facilitate learning

• Techniques to deal with fatigue, pain, & isolation

• Appropriate exercise• Appropriate use of medication• Effective communication with family, friends,

and health professionals• Nutrition
