National Patient Affinity Groups

• Affinity Groups include patient subject matter experts (SMEs) from all 18 Networks

• Patient SMEs chose to participate with one of the following Affinity Groups: – Bloodstream Infection (BSI) – Home Dialysis– Transplant– Improving Emotional Wellbeing

• Networks are mandated to implement patient-level educational resources developed or identified by the National Patient Affinity Groups in QIAs

Learning and Action Networks (LANs)

• A LAN is an improvement initiative that brings together healthcare professionals, patients, and other stakeholders to achieve rapid, wide-scale improvement– Learn more about LANs here:

• The 2019 LANs focus on:– Reducing BSIs– Increasing the number of patients who start home dialysis– Increasing the number of patients on a transplant waitlist– Improving patients’ experience of care– Addressing Population Health topics– Patient and Family Engagement

• The QIA LANs are hosted by the ESRD National Coordinating Center (NCC)

• Meetings are held every other month • Meeting invitations are sent to facilities from

the Network • Interventions identified in the LANs will be included

in the QIAs and distributed Network-wide • QIA facilities are required to attend their QIA topic

Sharing LAN Best Practices

LAN best practices are shared with all Network dialysis facilities via:• Bi-monthly LAN calls

– All facilities receive invitations

• Bi-monthly emails

• The LAN page on the Network website

QIA Interventions: Facility Level

The Network requires that facilities: • Conduct Root Cause Analysis (RCA)

• Provide staff education

• Report monthly to the Network

• Monitor and discuss progress of Network QIAs in monthly Quality Assessment and Performance Improvement (QAPI) meetings

• Identify facility level interventions that can be shared with other facilities

• As of July 8, 2019, facilities have recruited and registered 104 NPRs

• All NPRs and facility leads received an email from the Network with a list of activities to consider

• An NPR is an individual who:– Is on dialysis or who has had a

kidney transplant.– Volunteers at their facility or

a facility in their area.

• Facilities are encouraged to engage NPRs in the QAPI program

• NPR materials are available on the Network 7 website

Potential NPR activities

Consider using NPRs to:

• Provide the patient perspective for QIAs that the

facility is participating in with Network 7

• Pass out Network 7 educational resources or

newsletters to patients

• Participate in the facility’s monthly QAPI meetings

• Act as a greeter for patients that are new to the facility

• Support patients who are transitioning to home dialysis or are being worked up for transplant
