A HOPE: Avoiding re-hospitalization for heart failure

• SC1-BHC-08-2020: New interventions for Non-Communicable Diseases

• Project Description – aims to recruit a significant number of patients hospitalized for heart

failure, and evaluate the impact of using ICT on patient readmission, their clinical control and adherence to medical recommendations

– Current consortium is open (core Spain, UK and Slovenia)
