o Care Plans are personalized and automatically created based on claims data, onboarding questions and HRA responses.

o Care Plans give members health actions to complete to meet their health and wellness goals and manage their condition(s).

Based on current rulings, sweepstakes prize values that are awarded will contribute to the maximum allowable incentive for participatory and health contingent wellness programs. Blue KC recommends if you offer additional employer specific incentives, please review your design with your legal team to ensure compliance with current EEOC regulations.

Monthly sweepstakes offer a variety of gift cards. Members can use their points

earned to opt into sweepstakes drawings.
