Purpose of the Coaching Calls

Org Leads • Lead the calls• Help identify relevant topics• Address data submissions and trends• Troubleshoot, answer questions• Support facility team members• Be familiar with concepts, processes and tools• Facilitate teams’ access to project resources • Keeping them on track with project deliverables • Monitor individual facility and state- or regional-wide progress throughout the
duration of the project• Organizational Lead Manual available on www.ltcsafety.org
Expectations Roles Responsibilities
Facility Leads• Promote project goals • Lead the facility’s technical & cultural interventions• Engage in all educational sessions and coaching calls• Track facility progress and meet data requirements:• Meet regularly with LTCF team members to monitor progress • Ask for help on behalf of the team

Coaches• Expert resource• Provide direction and guidance and review facility reports and issues
HRET Advisors• Provide agenda templates, tools, and training• Support data management work • Help to identify relevant topics, address data submissions and trends and
troubleshoot and answer questions from the teams on calls.

Engage Facility Leads in Calls• Circulate each month’s agenda well in advance of the coaching
call date • Keep in touch regarding changes, cancelations, availability• Listen to your facility’s unit’s learning needs and create tailored
learning opportunities Encourage Call Participation
• Set Expectations• Follow-up with those unable to attend• Maintain open communication about topic interests• Leave time in presentation for questions, pose questions
