PG&E Customers:

Two forms must be sent in to Prime Opportunities Inc: the Six-Month Notice and the Authorization Form (which must be signed by your customer).http://www.energychoices.us

On the Six-Month Notice, the "Service Agreement Number" is the same as your electric Service ID Number, found on the Electric Detail Page of your PG&E bill.

PG&E requires the specific, electric Service ID #'s for corresponding accounts you intend to switch service be listed on the form. The form will be void if the utility finds the incorrect electric Service ID # on the form.

The Authorization Form requires the Service Account Number, not the Service ID #.PG&E requires that you initial box #1 and box #6 on page 1 of the Authorization Form. Please also initial the first box on page 2 of the form.Corrections/insufficient information will need to be corrected within 2 business days after submission or the Notice will be void.

Important Notes:Prime Opportunities Inc: will be submitting the Six-Month Notice and Authorization forms to the utility on your behalf via email and fax. Notices submitted to the utility via email will receive an email acknowledgment of receipt, typically within 30 minutes.Once the limit has been reached, customers will be notified and invited to begin submitting notices at the next available OEW, which may not be until 1/1/2011 for switching to DA in 2012.

Multiple Customer Accounts:Customers with more than one account wishing to switch to Direct Access should provide an attached Excel spreadsheet to accompany the Authorization and Six-Month Notice forms. This spreadsheet should include: Electric Service ID #'s (if PG&E)/ Electric Account #'s (if SCE/SDG&E), Name on Account (as listed on your bill), Service Address/City/Zip, and Customer Email address.

The utilities require we submit no more than one Six-Month Notice per Federal Tax ID # (per utility), and all customer accounts listed on the Notice must be for the same customer of record (i.e., same Tax ID #). Any service agreements not under the same Federal Tax ID will be rejected and must be resubmitted on a separate Notice.Multiple accounts should be listed in order of preference on the attached Excel spreadsheet.
